Increased early allograft dysfunction after donation after circulatory death liver transplantation: A single-center retrospective study.
Donation after circulatory death (DCD) is one of the most popular transplantation strategies for patients with end-stage liver disease, as the shortage of deceased-donor organs is a major obstacle for liver transplantation. However, the safety of DCD transplantation is still unclear. We retrospectively reviewed 59 DCD transplants performed from January 2008 to December 2015. DCD livers were procured by 4 different strategies: hypothermic machine perfusion (HMP), cold storage, in situ liver perfusion (ISP) followed by cold storage, and cold storage only. The median graft weight was 620 g. The median cold ischemic time was 5.5 hours. The total median follow-up duration was 1,892 days. The 5-year patient survival rate was 86.2%. Intraoperative red blood cell transfusion was required in 15 (25%) patients. The median total hospital stay was 34.5 days. Although the DCD grafts exhibited a trend of more severe ischemic injury, there were no significant differences in terms of 1-year patient and graft survival rate, survival free of retransplantation, and length of hospital stay. Furthermore, we found that early allograft dysfunction was more likely to occur in the DCD group than in the living donor liver transplantation (LDLT) group. Donor age and cold ischemic time were significantly associated with increased early allograft dysfunction. In conclusion, DCD is a feasible strategy for liver transplantation. However, DCD grafts are associated with increased early allograft dysfunction after transplantation.
